Outpatient Programs

TherEX Outpatient Therapy Programs are designed to improve the quality of life of patients by improving function and decreasing pain.

Our outpatient therapy consists of evaluation of:
 • patients’ limitations
 • pain and functional abilities
 • therapeutic interventions and modalities to restore function and decrease pain
 • on-going patient education.

Our therapists provide patients individualized, hands-on care and education in an outpatient setting. Additionally, therapists work closely with physicians providing updates and seeking input regarding progression of treatment.

Inpatient Medical Surgical

At TherEX, our primary goal is to work with the patient, physician and family members to determine the level of care needed to get patients back on track as quickly as possible.

During the initial phases of rehabilitation of an inpatient medical surgical environment, our therapists work with a team of health care professionals, including physicians, nurses, and case managers/social workers.

Rehabilitation services on an inpatient medical surgical unit consist of:
 • evaluating a patient’s limitations
 • care planning, including patient/family instruction  and education
 • movement training, initiation of exercise and gait  training
 • discharge planning to the next level of care along the rehab continuum

Acute Rehab Unit

Our TherEX approach to acute rehabilitation or Inpatient Rehab facility (IRF) provides patients with a supportive, structured and coordinated environment to rapidly progress a patient’s independence and function.

Acute rehabilitation programs are highly structured, intensive interdisciplinary programs intended for patients with significant functional impairments as a result of injury, illness, or trauma. Patients receive an average of three hours of therapy 5 to 7 days per week, depending on individual goals and expected discharges. During an acute rehab unit admission the plan of care is carried out 24 hours a day by therapists, rehabilitation nurses, social workers and physicians.

Skilled Nursing Unit

At TherEX, we assist a skilled nursing unit by providing interdisciplinary staff and individualized patient care plans. Our purpose is to facilitate functional independence, reduce dependent care and prepare patient and family for a successful return home..

The transition between a hospital stay and going home often leads a patient to a skilled nursing facility The rehabilitation team works with a full complement of interdisciplinary services with the goal of helping patients reach discharge goals of being independent at home or to the next level of care.

Rehabilitation services on a skilled nursing unit consist of:
     • evaluation of the patients’ limitations
     • care planning, including patient/family education and instruction
     • therapeutic interventions to improve functional abilities
     • discharge planning.

Home Health

Whether patients are recovering from a serious illness, injury or surgery, or learning how to manage a chronic health problem, TherEX Home Health therapy services help patients gain independence in the home environment.

Home Health therapists visit patients in their home environment, which is often the most functional setting for therapy. Patients are instructed not only on mobility and strengthening, but also on modifications and specialized equipment they can obtain to make their home more functional. Home Health therapists work with a multidisciplinary care team including nurses, social workers and home health aides, as well as physicians.
